This application is for those wishing to be licensed, ordained, or to become an Alliance member. Please ensure you have read the requirements prior to filling out this form.

If wishing to apply for both membership and licensure at the same time, please ensure you have selected both applications in the form below. You must be a member of the Alliance in order for your licensure application to be considered. Additionally, applications for ordination are accepted after a 2-year process of licensure, as outlined in the requirements.

All applications received prior to August 25, 2025 at 11:59 p.m. will be considered for the time of licensure and ordination at the Gatekeepers Conference (September 25-27, 2025).
